Patients are advised to contact their insurance company before an appointment to verify individual physician participation.

Above are the insurance plans our physicians are contracted with and considered to be a participating provider. These claims will be processed in-network. Patients are still held responsible for their annual deductible and coinsurance. If you do not see your plan listed above, you can still see our physician but your claim will be processed out of network and your individual participation may be higher.

If you have an HMO or POS plan it is the responsibility of the patient to obtain a referral from their primary care physician. If you are uncertain or if your plan requires a referral, contact your insurance company.

For surgeries performed in our office, most plans do not require precertification for surgeries performed in the doctor’s office. Check with your insurance company to see if precertification or prior authorization is required.
