If you have an HMO or POS plan it is the responsibility of the patient to obtain a referral from their primary care physician. If you are uncertain or if your plan requires a referral, contact your insurance company.
For surgeries performed in our office, most plans do not require precertification for surgeries performed in the doctor’s office. Check with your insurance company to see if precertification or prior authorization is required.
We accept traditional Medicare insurance plans, however, we do not participate in any Medicare managed care (HMO) plans.
