Patients are advised to contact their insurance company before an appointment to verify individual physician participation.
Above are the insurance plans our physicians are contracted with and considered to be a participating provider. These claims will be processed in-network. Patients are still held responsible for their annual deductible and coinsurance. If you do not see your plan listed above, you can still see our providers but your claim will be processed out of network and your individual payment portion may be higher.

If you have an HMO or POS plan it is the responsibility of the patient to obtain a referral from their primary care physician. If you are uncertain or if your plan requires a referral, contact your insurance company.
Most plans do not require pre-certification for surgeries performed in the doctor’s office. Check with your insurance company to see if pre-certification or prior-authorization is required.
In an effort to provide patients with fees that are fair, we offer self-pay fees based on Medicare's limited fee schedule for our area. This plan allows patients with a reasonable alternative to what otherwise might be restricted options for seeking dermatologic care. Self-pay fees must be paid by the patient on the date of service.
